Why do we say 'costs an arm and a leg'?

Possibly originates from portrait painting—artists charged more for portraits showing limbs (arms and legs) than just heads. Another theory suggests it's simply a vivid way to express extreme cost, implying you'd have to sacrifice body parts to afford it.
